Submit your nominations for the Athletic Hall of Fame

Use the forms below to nominate Yellowjacket alumni and friends to the UW-Superior Athletic Hall of Fame.

2) Complete the form and save a copy for your files.

3) Mail your forms and all supporting materials (photocopies of newspaper, yearbook or web page articles; letters of recommendation or testimonials; statistics; copies of photographs, etc.) to:

Individual Award -- This award is to recognize and honor individuals who have displayed distinctive, unique or exceptional athletic accomplishment at UW-Superior. Individuals must have graduated or completed a minimum of two years at UW-Superior, and must have completed competition a minimum of five years ago.

Team Award -- This award is to recognize and honor teams that have displayed distinctive, unique or exceptional athletic accomplishment at UW-Superior. Teams nominated must have completed competition a minimum of five years ago.

Carl Vergamini Award for Contributions to Yellowjacket Athletics -- This award is to recognize other accomplishments outside Yellowjacket competition. It will recognize individuals who have made positive contributions that support and promote Yellowjacket athletics. These individuals need not be graduates of UW-Superior.

Mertz Mortorelli Coaching Achievement Award -- This award will honor career achievement in coaching. Individuals nominated must display exceptional coaching abilities at any level. They must be graduates of UW-Superior.

Lydia C. Thering Meritorious Service Award -- This award recognizes an individual who has made substantial and innovative contributions to the principles of wellness in any society. These principles embrace (1) Personal responsibility (2) Sensitivity to environment (3) Nutritional awareness (4) Stress management (5) Physical fitness. The candidate is required to have attended UW-Superior a minimum of two years while participating in athletics for two seasons, and at least 10 years shall have passed since the individual last competed at UW-Superior.
