Budget Transfer Form
Use this form to transfer budget from one cost center to another. This forms applies to Funds 102, 104, 109, and 402.
 
Class A Raffle Approval Form and Sales Report
Please fill out this form if you wish to conduct a Class A raffle. A raffle where tickets are sold in advance of the day the drawing is conducted is considered to be a Class A Raffle. NOTE: ALL CLASS A RAFFLE TICKETS MUST BE APPROVED BEFORE TICKET PRINTING AND SALES OF TICKETS CAN TAKE PLACE. PLEASE CONTACT THE BUSINESS OFFICE IF YOU WISH TO CONDUCT A CLASS A RAFFLE. Please submit the sales report to the Cashier's Office with your deposit after the raffle is completed. This report must be completed as all raffles must be reported to the Wisconsin Gaming Commission in order to renew the University's raffle license. A LIST OF ALL PRIZE WINNERS ALONG WITH THE SALE REPORT MUST BE SUBMITTED TO THE BUSINESS OFFICE AFTER THE RAFFLE HAS TAKEN PLACE.

Fundraising Forms
Please fill out these forms if your organization wishes to hold a fundraising event. Please send the documents to the University Advancement Office (OM 237) after they are filled out. The Budget Office will grant approval for the event after it has been reviewed by the Advancement Office.
