In order to charge a lab fee or a special course fee for a class, the special course fee request form must be filled out and completed.  The Business Office will review the request to make sure it falls within UW System guidelines (Policy Paper G29) for special course fees, and will then forward the request to the Provost's office for final approval.  After the Provost's approval, the completed form will be forwarded to the Bursar for implementation into the Shared Finanacial System.
