Student Support Services Qualifying Form

This is the application for the SSS Program. Completion of this form doesn't automatically qualify you. You will be notified if you are accepted into the SSS program. Thank you for applying.

Membership Responsibilities

  • To meet with your SSS counselor a minimum of two times each semester and log a minimum of 3 hours using SSS. This requirement ensures that you remain eligible for services every year.
  • Maintain at least 6 credits (half time status).
  • To understand that your grades will be monitored through your transcript.
  • Understand that we share a list of SSS students with the Academic Advisement Office in order to receive any academic progress on SSS students. If we receive notice that you are having academic difficulty in any of your classes, we will contact you to assist you in developing a plan to get you back on course.
  • To have an exit interview with your SSS counselor prior to leaving UW-Superior
