Institutional Review Board Submission Form

The Institutional Review Board Submission form is designed for research projects that meet the expedited or full board review requirements. This form should be completed, with all required attachments included, and submitted to irb@uwsuper.edu for review by the Institutional Review Board. No research can begin until formal IRB approval has been received.

For student researchers: this form should be completed in conjunction with your faculty/staff advisor. The Institutional Review Board Submission form must be submitted to the IRB by the faculty/staff advisor. Forms submitted by students will not be reviewed.

Declaration of Exempt Status

The Declaration of Exempt Status form should be completed by those whose research was categorized as Exempt through the Determining Appropriate IRB Review Level survey. The information provided will be used by the IRB to determine if the research project does indeed meet the requirements for exemption from IRB oversight. Only the IRB has the authority to declare a research project is exempt. No research can begin until a formal IRB declaration of exemption has been received.

Course Certification Form

The Course Certification form should be submitted by faculty/staff who will be requiring students to complete research as part of a course requirement and for which the primary goal of the research is student learning in the methods and procedures of research.

Modification Request Form

When modifications need to be made to a research project approved by the IRB an IRB Modification Request form must be submitted to the IRB for review prior to implementing any changes in the research. The completed form, with all required attachments, must be submitted to irb@uwsuper.edu for a review in order to receive approval of the changes proposed.

IRB Approved Research Closeout Report

IRB approvals are valid for one year from the date of approval. Once your research has been completed you will need to submit an IRB Approved Research Closeout Report, a short web form, to signify you have closed out your research.

IRB Renewal and Continuing Review

IRB approval is valid for one year from the approval date. If research will continue past the one year point, including the analysis of data, it must receive continued approval from the IRB. In order to receive continued IRB approval submit the online IRB Renewal and Continuing Review form. This form must be submitted at least 30 days prior to the expiration of the initial IRB approval. Research cannot continue past the originally approved IRB project period without formal approval from the IRB.

Notification of Unanticipated Risks Involving Students or Others

If at any point during your research an unanticipated problem involving risks to subjects or others occurs this must be reported immediately to the IRB using the Notification of Unanticipated Risks Involving Subjects or Others form. See the Researcher Responsibilities page for more information.

Notification of Complaints Involving IRB Approved Research

If at any point anyone lodges a complaint regarding your research this must be reported immediately to the IRB, using the Notification of Complaint form.
