These forms apply to Fall 2025, Spring 2026 and Summer 2026 terms.
Adjustment Request Form – Use this form to request changes or corrections to your financial aid information or awards
Confirmation of Status (for Guardianship, Foster Care, Orphan, Ward of Court or Emancipation) – This form verifies your dependency status if you are – or were – under legal guardianship, in foster care, an orphan, a ward of the court, or legally emancipated.
Consortium Agreement FA – to be filled out by students who want their Financial Aid Office financial aid to cover courses taken at a different higher education institution (contingent upon approval). Please contact the Financial Aid office.
Graduation Plan (SAP) – Submit this form if you are following a Satisfactory Academic Progress (SAP) plan to complete your degree, outlining your remaining courses and expected graduation term.
Homelessness/Risk of Homelessness Confirmation – Use this form to verify that you are homeless or at risk of becoming homeless for financial aid purposes.
Housing Status Change – Report a change in your housing plans (on-campus, off-campus, or living with parents) so your financial aid can be adjusted if needed.
Non-Filer Status Reason – Complete this form if you (or your parent/spouse) did not file a federal tax return and need to explain the reason as part of the verification process.
Parental Non-Support – Use this form if your parents do not and will not provide financial support, allowing the Financial Aid Office to determine if additional documentation is needed.
Petition for Reinstatement of Financial Aid (all types) – to be completed if you are a student who has been put on Financial Aid Suspension.
Proof of Citizenship – Submit this form along with required documents to verify your U.S. citizenship or eligible non-citizen status for federal financial aid.
Release of Non-Directory Information – Use this form to give the Financial Aid Office permission to discuss your financial aid information with a parent, family member, or other third party.
Scholarship Notification – Complete this form to report any outside scholarships or funding you will receive so your financial aid can be accurately updated.
Statement of Educational Purpose – Use this form to confirm your identity and certify that any federal student aid you receive will be used only for educational expenses.
Total Permanent Disability Loan Discharge Certification – Submit this form if you are applying to have federal student loans discharged due to a total and permanent disability.
Waiver to Independency – Use this form to request a review of unusual circumstances that may qualify you to be considered an independent student for financial aid purposes.
WI Tuition Promise Work Attestation – This form verifies required work activity for students participating in the Wisconsin Tuition Promise program.
